Refreshing index doesn't reuse PBGitFile's

Steps to reproduce:
- Stage a file
- Change file
- Refresh index
- Stage new changes
there are now two files of the same name in the staged column

Fixed in my pu branch. Still need to do the same for untracked files though. Let's see if I can extract some of that cruft :)
